In the event of a member’s demise while traveling more than 75 miles from their residence as defined herein, HAUSA will facilitate and guarantee payment for all necessary arrangements and requirements in preparation for the transport and return of the member’s cremated remains, facilitate and instruct all necessary local crematory arrangements and documentation as required, while maintaining communications between the local sending crematory and service providers, transport carriers, your receiving service provider (if any) and the member’s family. HAUSA will also facilitate the procurement of consular services when necessary. HAUSA will wire required payment(s) to the HAUSA’s selected qualified service and transport providers for all of the above described arrangements up to a maximum of ten thousand dollars at no additional cost to Member. HAUSA will act as the member’s personal representative while maintaining necessary communication between all parties. In order to receive transport benefits described herein, HAUSA must be contacted first for all transport related arrangements to be facilitated by HAUSA through HAUSA’s selected qualified service provider network. If HAUSA is not notified first, HAUSA member benefits may not be covered, and additional costs will be incurred and no claims for reimbursement will be accepted by HAUSA. Benefits hereunder will not apply in the event of death in countries without diplomatic relationships with the United States or death caused by suicide, acts of war, terrorism death outside the United States while traveling for the specific purpose of receiving medical care or treatment outside of the United States. *Residence is defined herein and as anywhere a member resides for 90 days or more in the twelve months preceding the date of death. Qualification of member residence, USA Citizenship and next of Kin and/or member’s representatives’ will be required, verified and must be approved by HAUSA prior to facilitating arrangements by HAUSA. A nursing home, foster home or medical institution (or like kind) will be deemed the residence if the stay has exceeded 90 days. If enrollment in HAUSA occurs while traveling away from the member residence, HAUSA membership and benefits become effective for subsequent travel upon the Member’s return to their residence. Benefits under this membership are available only in countries with diplomatic USA relationship and only to legal USA citizens over the age of 21 residing in the United States.

STANDARD HAUSA MEMBERSHIP

‘STANDARD’ Membership $349.00 one Time Fee
For those who prefer cremation transport. Includes membership protection for your lifetime or ten years (whichever comes first) membership identification card, database entry, administration, access to HAUSA 24 hour response center, facilitation and payment of necessary crematory and transport arrangements through HAUSA network of qualified transport providers to return cremated remains in a minimum appropriate TSA required transport container urn to a location nearest the member’s residence, unless otherwise specified.

In the event of my demise while traveling more than 75 miles from my *residences, I authorize my representative and HAUSA or its agent(s) to facilitate all necessary crematory transport and return arrangements in accordance with this membership agreement. I understand that HAUSA is a professional transport facilitator and is not providing the actual funeral, cremation, merchandise or transport services which may or may not be needed into the future under this membership agreement. I also understand that my membership is not comprised of or represented as insurance or pre-paid cremation, funeral or cemetery arrangements of any kind. Amounts paid by the member herein will not be refunded after 30 days from the date herein. I also understand that my HAUSA Membership is transferable to a new residence in the USA, however it is my responsibility to notify HAUSA of such change of residence in order for HAUSA to revise its database information as necessary. I declare that I am over the age of 21 and a legal USA resident residing in the USA and agree to the terms and conditions herein.

YOU THE BUYER MAY CANCEL THIS TRANSACTION WITH FULL REFUND AT ANY TIME PRIOR TO MIDNIGHT OF THE 30th DAY AFTER THE DATE OF THIS TRANSACTION. NO EMPLOYEE OR REPRESENTATIVE IS AUTHORIZED TO MAKE VERBAL AGREEMENTS OTHER THAN THOSE CONTAINED HEREIN. YOUR MEMBERSHIP IS NOT INSURANCE OR A PREPAID FUNERAL OR CREMATION PLAN.
